Job Description

Hiring Bonus $500.00 Job Overview We are seeking a dynamic and compassionate Resident Care Coordinator to join our dedicated healthcare team. In this vital role, you will serve as the primary liaison between residents, families, and healthcare providers, ensuring comprehensive, personalized care plans are developed and executed effectively. Your leadership will foster a supportive environment that promotes resident well-being, safety, and dignity. This position offers an exciting opportunity to utilize your extensive clinical knowledge and management skills to make a meaningful difference in residents' lives across various care settings. Duties Coordinate and oversee daily resident care activities, ensuring adherence to individualized care plans and regulatory standards such as HIPAA and long-term care regulations Collaborate with interdisciplinary teams including nursing, therapy, social work, and medical staff to facilitate seamless communication and optimal patient outcomes Conduct thorough assessments of residents' physical and mental health status, including vital signs, medication administration, and medical documentation using EMR (Electronic Medical Records) systems like Cerner or eClinicalWorks Manage case documentation related to medical records, ICD coding (International Classification of Diseases), CPT coding (Current Procedural Terminology), and discharge planning processes Supervise nursing staff and care team members, providing guidance on nursing policy compliance, patient care procedures such as catheter care or tube feeding, and managing team performance Ensure compliance with state healthcare regulations, long-term care standards, and infection control protocols while maintaining high-quality patient care standards Facilitate training sessions on topics such as IV infusion techniques, sterile processing, basic life support (BLS), and wound suturing to enhance team competency Skills Extensive knowledge of medical terminology, physiology, anatomy, and diagnostic evaluation methods Proven experience working with individuals with developmental disabilities, dementia or memory care needs, geriatrics, behavioral health issues, pediatric populations including neonatal ventilator management or toddler care Strong background in critical & intensive care environments such as ICU or Level I Trauma Centers; experience with acute pain management and ventilator management is highly desirable Proficiency in EMR/EHR systems including Cerner, Athenahealth or similar platforms; familiarity with
ICD-9/ICD-10
coding standards for accurate documentation Skilled in patient assessment techniques including vital signs measurement, venipuncture (phlebotomy), catheterization, medication administration (including injections), specimen collection/processing Demonstrated leadership in supervising nursing teams or managing care facility operations; excellent managerial communication skills for managing multidisciplinary teams effectively Ability to interpret healthcare regulations related to long-term care facilities and ensure ongoing compliance with HIPAA privacy laws and state healthcare policies Competence in performing physical examinations, airway management techniques such as intubation or gastric lavage procedures; experience with medical imaging or sonography is a plus Join us if you're passionate about elevating resident care through expert coordination and compassionate leadership. $25.00 - $26.50 per hour Expected hours: 40.0 per week
