Summary of Job Duties:

Independent Contractor that provides comprehensive family support by connecting families, professionals, and community partners to Help Me Grow Florida services and resources. Conducts developmental and behavioral screenings, assesses family needs, coordinates referrals, and provides follow-up and advocacy to support positive child and family outcomes. Builds collaborative community relationships, maintains knowledge of early childhood systems and local resources, documents services and outcomes in STAR, and participates in ongoing training and professional development to ensure high-quality support for families.

Essential Job Duties and Responsibilities:
Care Coordination:

Respond to inquiries from families, early childhood professionals, educators, health care providers, and community agencies regarding child development, developmental concerns, and Help Me Grow Florida services.\ Administer and interpret developmental and behavioral screenings, including the

ASQ-3, ASQ

SE-2, M-CHAT-R/F, PSC, and PEARLS, to support early identification of childrens strengths and areas of concern. Assess family needs, provide individualized referrals, coordinate connections to appropriate services, advocate for families when needed, and provide ongoing follow-up to support successful linkages. Maintain comprehensive knowledge of Help Me Grow Florida services, early childhood systems, and community resources related to child development, developmental disabilities, and children with special health care needs, including eligibility requirements and referral processes. Build and maintain awareness of regional resources, programs, and service providers to effectively connect families with appropriate supports and reduce barriers to care.

Documentation :

Accurately document family interactions, referrals, services, and relevant case information within the System for Tracking Access to Referrals (STAR) database. Track, document, and report family outcomes, barriers to services, community resource gaps, and other challenges to support continuous quality improvement and program evaluation.

Professional Development & Collaboration :

Build and maintain collaborative partnerships with community organizations and service providers to support Help Me Grow goals and improve connections for families. Participate in local Help Me Grow meetings and community discussions to strengthen coordination, communication, and service delivery. Engage in ongoing training and technical assistance opportunities related to intake, developmental screening, referrals, outreach, and other core program activities. Maintain current knowledge of child development research, best practices, and emerging trends to support high-quality family engagement and referrals.

QUALIFICATIONS

Bachelors degree in special education, early childhood development, social work, psychology or related health or human services field OR associates degree in early child development or related health or human service field, plus two years of experience working with children and families Knowledge of services supports and systems that serve children and families, particularly Florida's system for early childhood services. Must reside in the Emerald Coast region. Ability to speak Spanish is highly desirable. Ability to communicate effectively verbally and write in a clear, concise, and effective manner. Ability to work effectively with people of diverse backgrounds and demonstrate cultural sensitivity. Knowledge of and respect for confidentiality involving clients, stakeholders, and fellow employees. Ability to establish and maintain effective working relationships with others. Ability to work independently and collaboratively with limited supervision. Ability to plan, organize, and coordinate multiple work assignments with attention to detail. Ability to solve problems and meet deadlines in a fast-paced environment. Skilled in software applications such as Microsoft Office Suite for word-processing, spreadsheets, and databases. Travel as needed. Position requires sitting, bending, and standing with light to moderate lifting usually not exceeding 20-25 lbs. Visual demands of the job include reading materials in written form and on computer monitors.
