It helps adults and youth find and connect with local mental health and substance abuse treatment providers. The McCabe Connections Center Team screens, refers, and assists identified community members to ensure access to mental health or substance abuse services as needed. The team will aid community members with other barriers that impact the social determinants of health. Summary of Position The Clinical Specialist works as part of the McCabe Connections Center Team. The Clinical Specialist will screen community participants using nationally normed screening tools and be the team lead for developing and monitoring the clinical screening program and protocols. The Clinical Specialist will assist the other team members with their respective specialties when necessary. The Clinical Specialist will build and maintain relationships with the full network of mental health and substance use providers in the community, developing a working knowledge of each provider's specialty and capacity in order to make appropriate warm referrals. will serve as the central point of contact for incoming clients, ensuring they are screened and connected to the appropriate provider in a timely manner, and that referral data and outcomes are tracked and fed back into the process. Essential Duties and Responsibilities
- Builds and maintains relationships with all mental health and substance use providers in the community, ensuring the full provider network is known and accounted for and can be engaged to create warm referrals for clients
- Maintains a nuanced, current understanding of each provider's specialty, scope, and capacity in order to match clients with the most appropriate provider
- Provides clinical screenings to clients with behavioral health and substance use conditions
- Is the team lead on various nationally normed clinical screening tools
- Communicates with community providers on the efficacy of utilized screening tools
- As part of a team, communicates and develops the care plan and serves as point of contact to ensure services are rendered appropriately
- Implements, coordinates, and monitors strategies for clients and families to improve health and quality of life outcomes
- Develops, documents, and implements the care plan which provides appropriate resources to address social, physical, mental, emotional, spiritual, and supportive needs
- Acts as an advocate for client's care needs by identifying and addressing gaps in care
- Facilitates a team approach to ensure appropriate interventions of quality of care and services across the continuum
- Provides assistance to clients with questions and concerns regarding care, providers or delivery systems
- Serves as a central point of contact for incoming clients, ensuring they are screened and referred to the appropriate provider in a timely manner
- Maintains and oversees data collection and reporting for referrals, as well as maintains and updates provider information
- Tracks referral and warm hand-off data and maintains a feedback loop with providers to confirm clients were successfully connected to and received services
- Maintains professional relationships with all stakeholders and establishes MOUs when necessary. Special Skill Requirements

- Proactively continues to educate self on providing quality care and improving professional skills Education/Experience
- BA in Human Services with commensurate training and experience
- Experience in clinical or community resource settings, clinical screening is desirable
- Highly organized with ability to keep accurate notes and records
- Local knowledge about and connections to community health care and social welfare resources is desirable
Physical Requirements:
With or without reasonable accommodation, employees in this position must be able to sit and type at a computer terminal for up to 8 hours per day with appropriate breaks and perform tasks that require fine dexterity and repetitive motions using the arms, hands, wrists, and fingers. Some functions require standing and sitting for extended periods of time. Lifting may be necessary at times. Must be able to stoop, bend, and lift on a frequent basis.
