LOCATION
Dublin, GA The Community Service Board of Middle Georgia is seeking a dedicated individual to join our team as a Mobilizing March Care Coordinator. Our Care Coordinators serve youth and families in need of customized, coordinated care to discover the underlying needs of the family by building upon the strengths of the family and their support. The Care Coordinator will also be responsible for engaging with assigned families and their formal and informal support to determine all needs and strengths. Care Coordinators use learned knowledge to create a team to produce innovative strategies to address the identified underlying needs of the individuals served. The Care Coordinator will work with all involved to ensure this team moves in the same direction, everyone stays informed and monitors needs and progress.
JOB DESCRIPTION/DUITES
Contact families referred to M2C within 24 hours of receiving the referral and schedule initial Hope Huddle and Realignment Meetings. Document efforts for failed attempts in EMR. Ask the next best questions to gather family stories including family strengths and needs. Complete initial Hope Huddle 14 days referral. Complete action plan for each assigned family within 30 days.. Maintain at least 1 ½ hours of focused time per family weekly, can include contact with family and/or supports. Form and guide an inclusive team to support the family including both formal and non-formal support. Promote innovative strategies to meet the unmet needs of families served. Communicate strategies and updates with all team members. Follow up with team to ensure strategies are being followed through. Work closely with assigned
CPS-P/CPS-Y
to understand their knowledge of the family and their ability to support the parent/youth. Attend all training offered through M2C, HFCC, and CSBMG. Accept directions and feedback from supervisors and follow through appropriately. Complete progress notes no later than 24 hours after delivering service. Will assist each youth with exploring expressive arts monthly. Will effectively communicate with schedulers and treatment team members weekly to ensure staff are scheduled, and everyone is working towards treatment goals. Here are some of the things we require: High School Diploma/GED Equivalent (All Bachelor level and unlicensed care coordinators must be supervised by a licensed mental health professional.) Minimum of two (2) years clinical intervention experience in serving youth with SED or emerging adults with a history of mental health Valid Georgia Driver's License Effective verbal and written communication skills Strong interpersonal skills and the ability to work effectively with diverse communities Ability to work independently and in collaboration with others Experience with Microsoft 365 Office Products Ability to organize, prioritize and meet deadlines accordingly Here's what would put you over the top: Bachelor's Degree in a related field (Social Work, Psychology, Human Services, Etc.)
