Caregiving/personal care experience preferred Overview Pure Hearts Personal Care Home is seeking a dependable, compassionate, and organized Resident Care Coordinator to support the daily care, safety, supervision, and well-being of our residents. The Resident Care Coordinator works closely with the Administrator/On-Site Manager, Supervising Registered Nurse, residents, families, physicians, pharmacies, and other healthcare professionals to help ensure residents receive consistent, person-centered care. This position may be filled as an employee or contractor, depending on the arrangement. Candidates who are properly trained and competency-validated may also perform approved Proxy Caregiver duties in accordance with Georgia requirements. Responsibilities Provide and coordinate assistance with activities of daily living, including bathing, grooming, dressing, toileting, eating, mobility, and personal hygiene. Assist with meals, snacks, hydration, laundry, light housekeeping, room organization, and maintaining a clean and safe environment as needed. Provide watchful oversight and maintain awareness of residents' needs, functioning, safety, and whereabouts. Help ensure residents are clean, comfortable, safe, properly supervised, and treated with dignity and respect. Monitor residents for changes in physical condition, behavior, cognition, appetite, mobility, skin condition, or overall well-being. Promptly report changes in resident condition to the Administrator, Supervising RN, physician, family/representative, or other appropriate parties as directed. Assist with resident appointments, transportation arrangements, pharmacy needs, supplies, follow-up care, and communication with authorized providers. Review and follow resident care plans, physician orders, dietary instructions, safety precautions, and facility procedures. Assist residents with self-administered medications within the limits permitted by Georgia law and facility policy. When properly trained, authorized, and competency-validated, perform approved Proxy Caregiver health-maintenance activities in accordance with the resident's written plan of care and informed consent. Accurately complete daily care records, service records, progress notes, medication documentation, incident reports, appointment records, and other required documentation. Coordinate resident care and direct-care activities during assigned shifts when designated as responsible staff. Assist with staff assignments, shift handoff, scheduled activities, meals, appointments, and resident routines. Respond appropriately to emergencies, falls, injuries, illness, fire, severe weather, missing-resident situations, and other safety concerns. Immediately report suspected abuse, neglect, exploitation, mistreatment, or resident-rights violations. Support resident independence, privacy, dignity, choice, community participation, and person-centered care. Assist with meals, snacks, hydration, laundry, light housekeeping, room organization, and maintaining a clean and safe environment as needed. Follow infection-control, emergency-preparedness, confidentiality, resident-rights, and safety procedures. Maintain professional communication with residents, families, staff, and healthcare professionals. Perform other duties as assigned that are within the individual's training, competency, authorization, and legal scope of responsibility. Proven experience working with individuals with disabilities, dementia care, or senior populations in assisted living or long-term care settings Strong knowledge of medical terminology, ICD coding (International Classification of Diseases), CPT coding (Current Procedural Terminology), and healthcare documentation standards Demonstrated proficiency with EMR/EHR systems such as Cerner or Athenahealth; experience managing medical records is essential Critical care experience including ICU or Level I & II trauma center backgrounds highly preferred Valid Basic Life Support (BLS) certification; proficiency in IV insertion, phlebotomy, catheterization, sterile processing and infection control practices required Excellent managerial communication skills for supervising team members and coordinating patient care effectively Familiarity with state healthcare regulations, HIPAA compliance, and long-term care regulations is necessary Ability to perform physical examinations, vital signs assessment, specimen collection/processing, and other clinical procedures confidently Experience working with diverse populations including pediatrics (neonatal ventilator use), adults in hospital medicine or home care settings preferred Requirements Must be 21 years of age or older for this supervisory/responsible-staff position. High school diploma, GED, or equivalent relevant work history in caregiving, personal care, assisted living, long-term care, home care, human services, or a related field. Previous experience working with elderly adults, individuals with disabilities, or residents requiring personal-care assistance is preferred. Must successfully complete required criminal background and/or fingerprint checks. Must meet applicable Georgia Personal Care Home health-screening requirements, including required physical examination and tuberculosis screening. Must maintain current CPR and First Aid certification, unless exempt as a currently licensed healthcare professional. Must complete all required facility and Georgia-mandated training. Must complete required annual continuing education. Must understand and respect resident rights, confidentiality, privacy, dignity, choice, and independence. Must be able to recognize and appropriately report changes in resident condition. Must have strong communication, documentation, organization, and interpersonal skills. Must be dependable, professional, patient, attentive, and able to work independently when necessary. Must be able to follow written care plans, physician orders, facility policies, and supervisory instructions. Must be able to respond appropriately during medical or facility emergencies. Must be physically capable of safely assisting residents with mobility and personal-care needs, with or without reasonable accommodation. Proxy Caregiver duties require additional resident-specific training, written authorization, informed consent, competency validation, and ongoing competency requirements before those duties may be performed. Must be willing to complete additional training when resident needs or assigned responsibilities require it.
