Overview of the role Provide care coordination to individuals who have had repeat interactions with the Law Enforcement Liaison team or law enforcement. Support clients in meeting their basic needs and connect them to resources, such as healthcare providers, community services, and social support systems. Foster and maintain positive relationships with community partners. Create and implement field-based case plans for 10-15 justice involved adults with behavioral health needs, in collaboration with clients. Take referrals for inmates being released from the Linn County Correctional Center who have behavioral health needs to develop short term stabilization plans upon release.
Schedule:
This is a full-time position that will require flexible scheduling, dependent on needs of clients and employee's availability. This may include work during daytime hours and may include some time into the evenings. Employee may work with the Program Supervisor/Manager to flex hours to meet required time. What you need Applicants must possess an A.A. of higher education. Human/social services field preferred. Minimum of 2-year professional experience in behavioral or mental health field required. Individual combinations of education and experience will be evaluated, and may vary for each candidate. Comfort using basic computer software. Basic typing and data entry skills. Valid driver's license and be insurable by the agency auto insurance policy. Pass all background checks completed by Foundation 2 Crisis Services. Submit copy of college degree or transcript as required by licensure for personnel file. Live within 50 miles of Cedar Rapids, IA Be at least 21 years of age. Successfully complete required trainings within required time periods. Bachelor's degree preferred. Experience in crisis services preferred. Education or training in trauma-informed care preferred. Experience in strengths-based practices preferred. Experience in conflict resolution and de-escalation techniques preferred. Ability to clearly communicate emotional support to people in crisis preferred. Background in motivational interviewing preferred. Individuals with current certification as a Peer Support Specialist/Peer Recovery Specialist in Iowa are encouraged to apply. Knowledge/experience working in the mental health, substance use, and/or problem gambling field preferred.
