Job Description
Griswold Home Care for Barrington & Palatine is seeking a dedicated and compassionate Lead Care Coordinator to join our team in Barrington, IL. In this role, you will be responsible for overseeing care coordination efforts, ensuring high-quality service delivery, and acting as a key liaison between clients, caregivers, and the company. - Coordinate and manage care plans for clients to ensure personalized and effective care.
- Serve as the primary point of contact for clients, families, and caregivers, addressing any concerns or special needs.
- Supervise and support care coordinators and caregivers to maintain high standards of service.
- Collaborate with healthcare professionals and community resources to enhance client care.
- Maintain detailed and accurate records of client information, care plans, and communications.
- Conduct assessments and follow-ups to monitor client progress and satisfaction.
- Assist in training and onboarding new care coordinators and caregivers.
- Ensure compliance with company policies and regulatory requirements. Requirements
- Proven experience in care coordination, case management, or a related healthcare role.
- Strong leadership and organizational skills with the ability to manage multiple tasks simultaneously.
- Excellent communication and interpersonal skills.
- Ability to work compassionately with clients and their families.
- Knowledge of local healthcare resources and community services.
- Proficiency in documentation and care management software is a plus.
- Valid driver's license and reliable transportation may be required.
