Job Description
Community Hope & Recovery Center in Beardstown, IL is looking for a passionate and forward thinking individual to join our team as the Family Care Coordinator. The Family Care Coordinator will work directly with client/families to assess their psychological, social and functional needs. Will be responsible for advocating and guiding families through complex care systems and collaborating with multidisciplinary teams to ensure holistic support. The role will focus on empowering clients and families and reducing barriers to care. Candidates at a minimum must posses a Bachelors or Masters degree in behavioral health field. Additionally be proficient orally and electronically, and be proficient in the use of computer and software applications. Reliable transportation and valid driver's license required. Must be able to pass all required background checks.
