Job Description

POSITION SUMMARY
The Social Services Coordinator is responsible for providing related social work services so that each resident may attain the highest practicable level of physical, mental and psychosocial well-being. The person holding this position is delegated the responsibility for carrying out the assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures.
DUTIES/RESPONSIBILITIES
Provides orientation to long term care facility and services rendered, to residents and family members Arranges inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement Schedules appointments with residents or responsible party, guardian, next of kin, sponsoring agency or agencies or lawful representative Review and interpret Resident's Rights, including facility Policy and Procedure implementation.
EDUCATION/EXPERIENCE
Either a Bachelor's Degree in Psychology or Sociology; a B.A. or M.A. in Social Work; or a Licensed Clinical Social Worker's Certificate
