Our residents have contributed so much to our communities, and it's our privilege to ensure their time with us is meaningful and special. We are looking for a MDS Coordinator to join our team. In this role, you'll work closely with residents, families, and our care team to create and guide individualized care plans. This position is key to making sure each resident's needs and goals are met with compassion, dignity, and respect. Responsibilities Complete and manage Minimum Data Set (MDS) assessments with accuracy and timeliness, in accordance with federal and state regulations, including Medicare and Medicaid guidelines. Partner with residents, families, and the care team to design personalized care plans. Communicate and educate staff to ensure care plans are understood and followed. Monitor resident care and make updates as needed to support the best outcomes. Keep accurate records and provide updates to residents and families. Support and guide staff in delivering excellent care. Collaborate with healthcare providers and agencies when needed to coordinate care. Requirements Bachelor's degree (required). Current and active RN license (preferred) or LPN license with equivalent experience. Strong communication skills with residents, families, and care teams. A compassionate approach and the ability to handle sensitive matters with professionalism. Experience in utilization review or a related field (preferred, but not required). Ability to work independently and as part of a team. Willingness to work occasional overtime, if needed. Effective communication skills for collaborating across multidisciplinary teams in inpatient or outpatient environments. Pay:
$37.00 - $42.00 per hour
