Care Coordinator - Visiting Angels Since 2011, Visiting Angels has been a trusted home care agency serving families in our community with compassionate, professional care. We're looking for a Care Coordinator to join our team and help us continue our mission. This is a full‑time role for someone who is both caring and organized, who enjoys a fast‑paced, challenging environment where every day you make a real difference in people's lives. You'll support our clients in their daily living, with keeping the details and processes they need to thrive. At Visiting Angels, we support each other through cross‑trained, collaborative roles that oversee caregivers in the field with our clients at home. We operate with a "next level" mindset for customer service and team support—both in the field and in the office—and our culture speaks to how we work together and how we serve our community. Key Responsibilities Respond promptly to client inquiries and referral sources, providing clear and professional service information Conduct client assessments (phone and in-person), and supervisory visits. Develop, implement, and update individualized Plans of Care Coordinate and manage caregiver schedules to ensure consistency, coverage, and quality matches Support caregiver recruitment, onboarding, orientation, and ongoing engagement Resolve client and caregiver concerns with professionalism and urgency Build strong relationships with clients, families, caregivers, and referral partners Participate in an on-call rotation to support after-hours needs Qualifications Experience in home care, healthcare coordination, or a related field preferred Strong communication skills, both written and verbal Excellent organizational, multitasking, and problem-solving abilities Ability to remain calm, flexible, and solution-focused in a fast-paced environment Proficiency with scheduling systems and Microsoft Office Ability to work independently and make sound decisions Valid driver's license and reliable transportation Why Join Visiting Angels Established, reputable agency with 15+ years of service in the community A next-level, team culture of care, focused on support, accountability, and growth Meaningful work that directly impacts the quality of life for our clients and families Compensation is salary plus performance-based bonus opportunities tied to growth and service excellence Physical Requirements Ability to sit, stand, walk, bend, and lift up to 50 lbs. as needed Ability to travel to client homes and office locations Apply Today Join a team where your work truly matters. Pay:
$45,000.00 - $49,000.00 per year
