Care Coordinator - Heritage Behavioral Health Center |
Decatur, Illinois Position:
On-Site at 151 North Main, Decatur, IL Full-Time | Monday-Friday, 8 AM-5 PM | Every Other Friday Off (Paid Wellness Day)
Pay:
High School Diploma or equivalent + 5 years of supervised mental health or social service experience: $23.00/hour Bachelor's Degree in
Human Services, Psychology, Social Work, or Education:
$25.00/hour

ABOUT HERITAGE BEHAVIORAL HEALTH CENTER
We're a Certified Community Behavioral Health Clinic serving individuals across a multi-county area in and around Decatur, Illinois. We believe every team member, no matter their position, plays a part in client care, and we take care of our staff so they can focus on taking care of others.
WHAT YOU'LL DO
As a Care Coordinator, you'll be the connective thread that keeps clients engaged in their care from first contact through successful treatment.
You will:
Ensure clients get connected to behavioral health treatment, primary care, specialty care, and community resources Follow up proactively with clients to track progress and troubleshoot barriers to care Coordinate with primary care teams, behavioral health providers, and community partners on appointments and treatment plans Educate clients and families on care continuity and available resources Document client progress and service engagement in electronic health records Help clients navigate healthcare and social service systems Participate in interdisciplinary team meetings and ongoing trainings
WHAT YOU BRING
High School Diploma/GED plus 5 years of supervised mental health or social service experience, OR a Bachelor's degree in social work, psychology, education, or a related human services field Minimum age of 21 Valid driver's license, reliable transportation, and proof of auto insurance Strong communication skills and the ability to build rapport with diverse clients Comfort with electronic health records (EHR) and Microsoft Office A collaborative, adaptable, and empathetic approach to client care
