Job Description

Macon County Health Department | Decatur, IL Make a Difference Every Day. Are you passionate about helping older adults remain safe, healthy, and independent in their own homes? The Macon County Health Department is looking for a compassionate, organized, and community-minded Care Coordinator to join our Starting Point team. In this role, you'll build meaningful relationships with seniors and their families, connect them with vital community resources, and provide case management services that help delay or prevent unnecessary nursing home placement. If you're looking for a career where your work truly impacts lives, we'd love to hear from you. What You'll Do Conduct assessments and develop individualized care plans for adults age 60 and older. Coordinate services with healthcare providers, community agencies, and family members. Connect clients with local resources and supportive services throughout Macon County. Monitor client progress and adjust care plans as needs change. Maintain accurate documentation and case records. Advocate for clients while promoting independence, dignity, and quality of life. Build positive relationships with community partners and multidisciplinary teams. What We're Looking For Required Qualifications Bachelor's degree in a related field or Registered Nurse (RN). At least one year of experience working with community agencies or human services. Knowledge of community resources available in Macon County, particularly those serving older adults and individuals with disabilities. Strong communication, organization, critical thinking, and decision-making skills. Ability to work independently while managing multiple priorities. Passion for serving diverse populations with professionalism, compassion, and respect. Requirements Because this position serves clients throughout the community, applicants must have: A dependable personal vehicle available during work hours. A valid Illinois Driver's License. Current automobile insurance. Ability to travel throughout Macon County. Ability to enter and exit a vehicle multiple times each day in various weather conditions. Ability to safely lift, carry, pull, or transport up to 30 pounds of equipment.
Compensation Salary:
