As our Client Transition Coordinator, you'll be the face of Open Arms Solutions to skilled nursing facilities, rehabs, and senior living communities across the near western, and western suburb area — building the partnerships that connect families to the care they need, exactly when they need it. What You'll Do Build and grow relationships with social workers, discharge planners, directors of nursing, and resident services directors at skilled nursing facilities, rehabs, and CCRCs Visit assigned facilities regularly to represent Open Arms Solutions and educate staff on our private duty senior care services Turn referrals into new clients by coordinating smooth handoffs with our intake and care team Develop, host and attend community events, health fairs, and networking functions to expand our referral base Track your outreach and results, and hit monthly referral and admission goals What We're Looking For 2+ years in social work, home care, home health, hospice, or senior living business development or marketing (preferred) A natural relationship-builder who's comfortable walking into a facility and starting a conversation Strong communication and presentation skills Familiarity with private pay, Medicaid waiver, or long-term care insurance funding for home care (a plus) Reliable transportation and a valid driver's license — this role is out in the community, not behind a desk High school diploma required; Bachelor's degree preferred What You'll Get Competitive base salary plus performance-based incentive pay Mileage/travel reimbursement Full-time benefits A growing, founder-led company where your work directly shows up in results If you know how to build trust with healthcare professionals and want to be part of a company that's expanding across the western suburbs, apply today.
