Role:
As a Client Care Coordinator with SYNERGY HomeCare of Peoria, you will serve as the primary point of contact for our clients and their families. Your primary responsibility will be to coordinate client care services, ensuring that our clients receive the highest level of care and support. Your exceptional communication skills, attention to detail, and ability to work in a fast-paced environment will be essential in maintaining a high level of client satisfaction.
Responsibilities:
Coordinate client care services, including scheduling, managing, and delivering care to clients Develop and maintain positive relationships with clients, family members, and healthcare professionals Communicate effectively with clients, caregivers, and other stakeholders to ensure seamless care transitions Identify and address potential care gaps or concerns, working in collaboration with our care team to develop personalized care plans Conduct intake assessments, care planning, and ongoing assessments of client needs Stay up-to-date with industry regulations, policies, and best practices to ensure compliance and quality of care
Ideal Candidate:
Has a strong background in healthcare, social services, or a related field Possesses excellent communication, organizational, and problem-solving skills Is able to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities Has a passion for delivering exceptional client care and building strong relationships
