The Wound Care Program Coordinator will oversee the day to day operations of the wound care program. This includes monitoring trends in the facility in regard to the progress and treatment of wounds. The Wound Care Program Coordinator actively coordinates wound prevention, as well as provide consultation, educational support, and staff development activities that provide nursing staff with the knowledge, skills, and confidence to assist in the management of patient wounds. Role and Responsibilities Below are the roles and responsibilities that the Wound Care Program Coordinator is expected to fulfill to help facilitate quality, patient-centered care:
  • Oversee completion of wound assessments on all wounds.
  • Identify and monitor patients at high risk of developing pressure ulcers in the facility.
  • Manage completion wound risk assessments per facility policy and procedure
  • Work collaboratively with the interdisciplinary care team to facilitate achievement of desired treatment outcomes.
  • Round with wound care physician weekly.
  • Complete wound care treatments as ordered and delegate stable wound care treatments to floor nurses as appropriate using daily list for staff and communication with the weekend treatment nurse.
  • Assess patients upon admission for any skin breakdown and risk for skin breakdown.
  • Ensure wound notes are comprehensive and describe the patient and their wound in entirety, including pertinent medical history, admission date, plan of care, other wounds, barriers to healing, preventative devices, family support, patient/staff/family interview, and overall plan of care.
  • Assess each wound care patient for preventative devices, write orders as appropriate, initiate, and keep track of facility-wide preventative devices.
  • Create weekly list for wound physician based upon type of wounds (complexity, facility-acquired, multiple wounds) as well as carrying the patient from start to finish. Complete paperwork for wound physician and round with wound physician weekly.
  • Monitor TARS and wound treatment orders for errors. Correct and make changes as appropriate.
  • Ensure cost effective, appropriate utilization of wound care dressings, supplies, and pressure reduction specialty beds
  • Provide training, guidance, and counseling as needed to help improve staff performance.
  • Assist staff in providing hands on care to patients on the unit as needed.
  • Review treatment orders for completeness and accuracy on admission, weekly, and prn.
  • Meet with patients and families to assess needs and provide education on wound care management.
  • Ensure infection control practices are maintained by staff.
  • Other duties as assigned. Qualifications and Education Requirements
  • Current licensure as a registered professional nurse in the State of Illinois.
  • Degree from an accredited college or university nursing program.
  • At least two to five years professional nursing experience in medical/surgical nursing. Successful progressive supervisory/management experience preferred.
  • Requires knowledge and skills related to medical/operational systems to successfully maintain excellent nursing care.
  • Wound care certification required.
  • Strong communication, organizations, and leadership skills.
  • Self-motivated and able to work both independently and in collaboration with others.
