Job Description
Senior Services Associates, Inc. is a non-profit agency dedicated to sustaining and improving the quality of life for mature adults, persons with disabilities and their caregivers by providing access to the social services they need. We are dedicated to preserving their independence, promoting mental and physical well-being, and protecting their rights and dignity. We are seeking a full-time Care Coordinator- Bilingual, Spanish for our Yorkville Office . The Care Coordinator will be responsible for completing comprehensive community based assessments in the home for seniors in need. Candidate must possess a minimum of a BA/BS in social work, psychology, nursing or other Human Service related field or equivalent experience working in the Senior Community in a similar position.
Duties include:
- Completion of needs-based assessment in participant's home.
- Developing a Plan of Care for participant that addresses needs identified during assessment process.
- Referring client to internal/external resources.
- Yearly redetermination of eligibility for various programs.
- Assistance with completion of benefits applications including but not limited to Medicaid and SNAP.
- Complete accurate billing paperwork and documentation.
- Travel to and from office and participant's home.
- Regular travel for required/mandatory trainings.
- Candidate will develop a knowledge and understanding of Medicaid and Medicare to best serve our seniors.
Requirements:
- Candidate must be bilingual (speaking, reading, writing) in Spanish .
- Candidate must have a reliable vehicle and proof of insurance upon hire.
- Candidate will have ability to maintain professionalism in working with seniors and families in the Community and Office setting. Must maintain confidentiality of client information.
- Candidate must have proficient computer skills and a working knowledge of Microsoft Office Programs.
- Candidate must be able to manage-time efficiently and independently.
- Candidate must understand professional ethics and comply with company confidentiality policy.
- Candidate must have an understanding of non-profit agency and have a willingness and dedication to the Senior Service Mission. Volunteerism is seen as a great PLUS!
- Experience with seniors a plus.
Education:
- Bachelors degree is required.
- Candidate must possess a minimum of a BA/BS in social work, psychology, nursing or other Human Service related field or equivalent experience working in the Senior Community in a similar position.
