Job Description
Lafayette Franciscan Healthy Living Clinic 1116 N 16th St Lafayette, Indiana 47904 The Home Care Coordinator is an important role within the Franciscan Health team. This position is responsible for the coordination and implementation of home care services. These home care services extend to post-acute program participants.

QUALIFICATIONS
High School Diploma/GED- Required Bachelor's Degree
- Preferred 1 Year Working with Frail or Elderly
- Required 2 Years Home Care
- Preferred
