As a Discharge Planner , you will be responsible for initiating discharge planning for patients, working closely with the treatment team to schedule appropriate after care appointments and communicating with referral sources. You will coordinate, conduct, and implement discharge planning for patients at the levels of care being provided upon discharge. This role also works as a part of the treatment team and maintains effective communication with teammates and community resources and accountability for services provided.
