Job Overview:
The Life Skills Coach & Care Coordinator provides direct services to individuals with brain injuries while coordinating care, supporting Specialists, and strengthening Thrive Therapy's presence throughout Southeast Kansas. This position requires a self-motivated professional who enjoys building relationships, solving problems, and collaborating with both internal teams and community partners.
Responsibilities:
Provide one-on-one life skills coaching in clients' homes, community settings, and therapy centers. Teach and reinforce cognitive, behavioral, and independent living skills. Support clients in achieving individualized goals and increasing independence. Maintain accurate documentation, progress notes, and behavioral data. Develop, implement, and monitor client Plans of Care in collaboration with the clinical team. Provide case management services by coordinating care, communicating with clients, families, providers, and community resources to ensure continuity of services. Monitor client progress, identify changing needs, and coordinate appropriate interventions and resources. Build and maintain relationships with community partners and referral sources throughout Southeast Kansas. Important Information Before Applying This is a community-based rehabilitation, care coordination, and case management position . The Life Skills Coach & Care Coordinator provides direct rehabilitation services while coordinating client care, developing and monitoring Plans of Care, and collaborating with families, providers, and community resources to ensure continuity of services. While experience providing direct support is highly valued, this is not a CNA, home health aide, or personal care attendant position. This role focuses on coaching, care coordination, cognitive and behavioral rehabilitation, and helping individuals achieve greater independence. This position does not typically involve bathing, toileting, medication administration, skilled nursing, or other medical procedures. Travel Requirements This position serves clients throughout Southeast Kansas (SEK) and requires travel between client homes, community locations, and partner organizations. A company vehicle will be provided for work-related travel. Applicants must possess a valid driver's license and maintain an acceptable driving record. Qualifications Required Master's Degree in Psychology, Social Work, Counseling, Human Services, Rehabilitation, Healthcare Administration, or a closely related field. Excellent communication and relationship-building skills. Strong leadership, organizational, and time-management abilities. Ability to work independently while managing multiple priorities. Valid driver's license with an acceptable driving record. Preferred Experience working with individuals with brain injuries or neurological conditions. Experience in case management, care coordination, rehabilitation, behavioral health, or community-based services. Leadership or supervisory experience.
Pay:
$25.00 - $30.00 per hour
