Location:
Tri-County Area (Based out of Newton, KS)
Schedule:
Monday - Friday, 8:00 AM - 5:00

Full-Time Join our team at Prairie View, Inc. to support individuals in addiction recovery by coordinating their care, fostering their growth, and connecting them with vital community resources as part of our innovative new Mobile Care Unit ! About the Mobile Unit & Role As the Care Coordinator on our Mobile Unit, you won't be confined to a traditional office setting. You will travel alongside a close-knit, multidisciplinary team consisting of a Licensed Addiction Counselor (LAC) , an Advanced Practice Registered Nurse (APRN) , and a Peer Mentor . Together, your team will deliver direct support to locations throughout the tri-county area. In this role, you will serve as the hub of client support—assessing needs, developing treatment plans, managing crisis interventions, and connecting adult and adolescent clients to key community resources to build lasting autonomy and recovery capacity. Key Responsibilities 1. Direct Client Care & Case Management Provide continuous, supportive care coordination and crisis intervention for adult and adolescent clients in addiction recovery. Develop, facilitate, and monitor initial and periodic treatment plans (every 90 days), collaborating with clinical staff for oversight. Assess client strengths and needs to promote community integration, relapse prevention, and autonomy. Educate clients on service choices and skill-building for managing emotional disturbances. Build strong relationships with local agencies to create a seamless community support network. Maintain safe environments and professional boundaries using risk-factor and intervention strategies. 2. Administrative & Compliance Duties Maintain thorough, timely clinical documentation in compliance with agency and state licensing standards. Coordinate with Utilization Review staff to ensure proper service authorization and treatment plan accuracy. Manage daily scheduling and prioritize caseload tasks based on client and agency needs. Participate actively in team meetings, departmental briefings, and clinical supervision. 3. Professional Development & Quality Complete required State of Kansas trainings, including Person-Centered Case Manager (PCCM) certification. Fulfill annual continuing education requirements, workshops, and in-services (e.g., Infection Control). Participate in Quality Assurance & Performance Improvement (QAPI) activities to uphold care standards. 4. Teamwork & Organizational Values Safely transport clients as needed to facilitate service delivery and resource access. Foster a respectful, positive work culture while upholding client confidentiality and Prairie View's mission.
POSITION REQUIREMENTS
Experience:
Minimum of two years working with adults who have substance use disorder and/or severe and persistent mental illness.
Certification:
Must hold or be willing to obtain and maintain the State of Kansas Person- Centered Case Manager Certification (PCCM), including required Continuing Education Units (CEUs).
Skills:
Excellent verbal, written, and listening communication skills. Ability to work effectively and provide support to adults with substance use disorder/mental illness. Proficient at basic computer and keyboarding skills.
General Requirements:
Must be at least 21 years of age. Must be able and willing to work a flexible schedule to meet patient and organizational needs. Must pass required background checks (KBI, Central Registry, and DCF/SRS).
