Director of Assisted Living Position Summary for the
Assisted Living Clinical Care Coordinator:
The Assisted Living Clinical Care Coordinator is the nurse leader ( Licensed Practical Nurse or Registered Nurse) for Village Shalom's assisted living. As the lead nurse, the AL Clinical Care Coordinator applies comprehensive knowledge of nursing practices and procedures for assisted living. Provides guidance, training, and supervisor of CNAs, CMAs and Nurses in Assisted Living as needed. Develops and updates Negotiated Services Agreements and Health Services Plans for assisted living residents. Conducts daily clinical rounds and audits. Coaches charge nurses and house supervisors on standards of practice for Assisted Living nursing documentation. Performs quality improvement responsibilities through assessment, improvement and evaluation of service delivery. Documents and reviews medication and treatment sheets for accuracy and compliance with physician orders, professional standards, federal and state regulations and company policy. Documents information regarding nursing care, care plans, and observations of the residents' overall condition and behavior. Attends training to stay current with standards of care. Benefits for the
