Healthcare Coordinators play a vital role in supporting the coordination of our client's medical appointments and follow-up care. Healthcare Coordinators act as a liaison between the client's physicians and the client's support team. Many of the daily tasks in the medical department are dependent on the efficient communication and performance of the Healthcare Coordinators. Routine duties include prompt communication on all healthcare appointments, maintaining client's records/documentation, scheduling medical appointments, accompanying clients for emergency treatment, assisting with laboratory tests, assisting with client admission/discharge from outside facilities, routine communication with the pharmacy, and following up as various administrative tasks. Key Responsibilities of a Healthcare Coordinator Perform administrative duties Transport clients to and from medical appointments Advocate for proper medical treatment Respond to basic medical questions and concerns to provide informative support to the clients direct support workers. Document and communicate medical visits with appropriate individuals Report new diagnosis and/or medical concerns Monitor plans of action for follow up care Complete the process of patient admission and discharge from outside facilities Schedule appointments for clients and communicate with the client's support team Oversee proper documentation of vital signs, temperature, blood pressure, pulse, weight, etc. Maintain a healthy relationship with the clients and their support team Educate clients, parents/guardians on various medical information as needed Inventory of medical equipment Communicate with the pharmacy and place refills accordingly Assist with laboratory testing, including Covid-19 testing Complete reports and documentation within the client charting system Accompany clients for emergency treatment Adhere to all organizational standards, practices, and policies Ability to manage time effectively and productively Perform other duties as assigned Report directly to the Medical Program Manager Qualified applicants must have the following: A license/certification in one of the following fields: CNA, CMA, MA, EMT or LPN. Strong desire, compassion, and ability to help individuals with intellectual and developmental disabilities. Capable of working effectively with a team Demonstrate effective communication skills, both written and verbal Valid driver's license and the ability to operate one of our company vehicles, which include mini-vans, 12 and 15-passenger vans, and wheelchair lift vans Eligible to work in the United States At least 18 years of age High school diploma or GED Reliable transportation to and from work Successfully pass criminal background checks and random drug screens The ability to physically assist and transfer individuals to/from wheelchairs from/to other seating and beds The ability to lift a minimum of 50 pounds The ability to stand, walk and/or bend and squat for 6-8 hours The ability to utilize a desktop computer effectively for documentation and record-keeping The flexibility to work a shift or location other than the one you were initially hired to work You must have the ability to utilize the internet to access your personal e-mail and to view and confirm your schedule when posted You will be required to be at work when scheduled and work your entire shift
