Primarily weekdays with occasional evenings and weekends as needed Help residents and families navigate the transition from rehabilitation to home with confidence. At Masonic Homes Kentucky, our Social Services team plays an essential role in helping residents and families navigate care, recovery and the transition to their next level of support. As a Social Services Coordinator, you'll work closely with residents on our short-term rehabilitation unit, serving as a trusted point of contact throughout their stay and coordinating the many details that contribute to a safe, successful discharge. You'll join a close-knit three-person Social Services team and collaborate with nursing, therapy, providers, families and community partners to ensure residents have the resources and support they need. Why you'll love working here meaningful work supporting residents and families during important healthcare transitions opportunity to build relationships and make a direct impact on each resident's experience close-knit Social Services team with strong interdisciplinary collaboration variety in your day through resident support, discharge planning and care coordination stable nonprofit organization with a long-standing reputation for excellence comprehensive medical, dental and vision benefits 403(b) retirement plan paid time off educational and professional development opportunities What you'll do manage an assigned caseload while providing psychosocial support to residents and families complete social histories, assessments and individualized care planning documentation coordinate comprehensive discharge plans for short-term rehabilitation residents identify and arrange appropriate placement, home health and caregiver services to support safe transitions coordinate physician follow-up appointments and other post-discharge services arrange necessary medical equipment and supplies including oxygen, wound vacs, nebulizers and other resident-specific needs coordinate referrals for services such as home dialysis and hospice care facilitate care plan meetings with residents, families and members of the interdisciplinary team serve as a primary point of contact for residents and families throughout their stay provide timely updates and ongoing support related to care concerns, treatment and discharge planning collaborate closely with nursing, therapy and other departments to ensure coordinated resident care maintain accurate and timely documentation in the electronic medical record communicate resident updates and necessary information to insurance companies participate in weekly interdisciplinary meetings and provide current information regarding residents and discharge plans support residents and families with discharge notices and related documentation What you'll bring bachelor's or master's degree in social work or a related human services field 1-3 years of social services experience in skilled nursing, healthcare or long-term care preferred experience with short-term rehabilitation and discharge planning strongly preferred understanding of community resources, post-acute services and healthcare transitions strong organization and time management skills with the ability to manage a high-volume caseload excellent communication and relationship-building skills ability to confidently communicate with residents, families, healthcare professionals and community partners strong documentation skills and experience working with electronic medical records collaborative approach and ability to work effectively as part of an interdisciplinary team compassionate, resident-focused approach to supporting older adults and their families successful completion of pre-screening hiring requirements About Masonic Homes Kentucky Masonic Homes Kentucky is a trusted leader in senior living and healthcare services, providing compassionate care and innovative programs that enrich the lives of older adults.
