Aftercare Coordinator Marlborough Hills Rehabilitation & Health Care Center - 4.7 Marlborough, MA Job Details Full-time $23 - $25 an hour 10 hours ago Benefits Dental insurance Vision insurance 401(k) matching Employee discount Opportunities for advancement Life insurance Qualifications English Full Job Description We are hiring an Aftercare Coordinator to join our dynamic care team at Marlborough Hills Rehabilitation & Health Care Center! 8a-4p schedule with weekly pay. 32 hours a week. $23 to $25 an hour. The Aftercare Coordinator provides planning for residents with substance use disorders in preparation for their discharge.
Experience & Education:
Preferred associate's degree in human services and or other relevant area of studies in this field. Experience in aftercare coordinating and community support services preferred.
Duties & Responsibilities:
Assess resident needs, situations, strengths, and support networks to determine their aftercare goals. Develop aftercare plans to improve the residents' well-being. Help residents work with government agencies to apply for and receive benefits. Develop and implement aftercare plans which are measurable and appropriate for the identified level of care. Check-in with discharged residents.
Specific Requirements:
Must be able to read, write, speak, and understand the English language. Must possess the ability to make independent decisions when circumstances warrant such actions. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public. Must have patience, tact, cheerful disposition, and enthusiast, as well as being willing to handle residents based on whatever maturity level in which they are currently functioning.
