Job Summary We are seeking a compassionate, organized, and qualified Adult Foster Care (AFC) Care Manager to join our team. In this role, you will work closely with elders, adults with disabilities, caregivers, and registered nurses to oversee and support safe, high-quality care in qualified residential settings. The AFC Care Manager is responsible for conducting member evaluations, assisting with care planning, providing oversight to caregivers, and ensuring full compliance with Massachusetts AFC program regulations.
Conduct initial and ongoing evaluations of AFC qualified settings. Review and document fire and safety procedures, and evaluate setting suitability at least annually.
Psychosocial Evaluations:
Obtain comprehensive social histories and complete initial and ongoing psychosocial evaluations for members.
Care Planning:
Collaborate with the Registered Nurse (RN), members, and families to develop, implement, and conduct ongoing reviews of member AFC Plans of Care.
Caregiver Oversight:
Provide continuous oversight, clinical guidance, and support to AFC caregivers in conjunction with the RN.
On-Site Visitation:
Perform regularly scheduled on-site visits.
Documentation & Progress Notes:
Complete detailed care-management progress notes following each encounter, visit, or significant change. Review AFC caregiver daily logs at minimum every 30 days.
Community Linkage & Advocacy:
Assist members and caregivers in identifying, obtaining information on, and accessing needed healthcare and community resources.
Transition & Discharge Planning:
Participate actively in discharge and transition planning and implementation when care level changes occur. Qualifications & Requirements Candidates MUST meet one of the following two regulatory qualification pathways under MassHealth regulations: Option 1 (Licensed Social Worker): Bachelor's degree from an accredited institution. Active Social Worker License from the Massachusetts Board of Registration in Social Work (LSW, LCSW, or LICSW). Minimum of 2 years of recent experience working directly with elders or adults with disabilities. Option 2 (Clinical Experience): Bachelor's degree from an accredited institution. Minimum of 2 years of direct clinical experience in the care of elders or people with disabilities.
Additional Requirements:
Valid driver's license, reliable vehicle, and active auto insurance (required for field visits). Strong documentation, organizational, and time management skills. Ability to work effectively within an interdisciplinary healthcare team.
