Job Description
Behavioral Health Network (BHN) is a leading statewide provider of behavioral health services and a Certified Great Place to Work. With deep roots in Western Massachusetts and a growing presence in communities throughout the state, we are currently seeking a Care Coordinator II to join our Adult Community Clinical Supports (ACCS) team and help advance our mission of empowering people to live healthier, more fulfilling lives.

What You Will Do:
Provide direct interventions with individuals served to complete comprehensive assessments, treatment plans, and clinical interventions Provide modeling and clinical oversight for outreach and GLE direct care staff Conduct and oversee clinical services set forth by the Integrated Team, performing functions reserved for the Licensed Practitioner of Healing Arts (LPHA) as specified by contract Oversee services provided to all individuals served by ACCS Ensure the program adheres to clinical regulations and funding source requirements Produce documentation that is clear, legible, and meets DMH and ACCS standards Develop treatment plans consistent with evidence-based and best practicesWho You Are:
Master's Degree in a mental health-related field required Clinical licensure required (e.g., LCSW, LICSW, LMHC) Minimum of 2 years' experience in the human service field Committed to the empowerment and growth of individuals served, human rights, and community integration Knowledgeable in the interface between mental health and medical conditions, substance abuse, care management, recovery principles, evidence-based practices, and trauma-informed treatment Strong verbal and written communication skillsSupervisory Responsibility:
