If you're a CNA or GNA who has spent years giving great care, you already know what makes a shift go right and what makes it fall apart. You've seen the missed handoffs, the caregiver who wasn't introduced to the family, the schedule that only worked because someone remembered everything. You've probably thought, \"I could run this better.\" This is your chance. Rising Star Home Care Services is a Maryland licensed Medicaid and private pay home care agency serving clients in every county across the state through the Community First Choice and Community Personal Assistance Services programs. We are small, independently owned, and we have one goal: to be the best home care agency in Maryland. Not the biggest. The best. The agency families recommend to their neighbors and the one caregivers don't want to leave. We're honest that we're still building. That's exactly why this role matters. About the schedule No weekends. No overnights. No 12 hour shifts. You work 10:00 AM to 4:00 PM, Monday through Friday, and your mornings and evenings are yours. We designed it that way on purpose. What you'll do Build and manage the master schedule so every client has the right caregiver for every authorized hour Handle call outs and fill open shifts quickly so no client goes without care Visit client homes to introduce new caregivers and check in with families Screen caregiver applicants, run in office application meetings, and make sure every file is complete before anyone starts Correct missed clock ins and clock outs in Maryland LTSS the same day they are flagged Track caregiver CPR, training, and certifications, and follow up before anything expires Help coordinate client assessments with our nursing team Write down how things get done so our processes get better every month What we're looking for Active CNA or GNA certification At least one year of hands on experience in home care, home health, assisted living, or senior care A process mindset. You like checklists, clean files, and schedules that actually work, and you notice problems before they grow Someone who follows through. When you say it's handled, it's handled Comfort with scheduling software and electronic visit verification (Maryland LTSS experience is a big plus) Warm, clear communication with caregivers, clients, and families Valid driver's license, reliable vehicle, and willingness to travel to client homes Ability to pass a criminal background check Nice to have Experience with Medicaid funded services such as CFC or CPAS Familiarity with OHCQ standards Experience with an applicant tracking system like CareerPlug Some college coursework in health, human services, or a related field Your path to grow with us We would much rather promote the person who built the operation than hire over them. This role has a written career ladder: Care Coordinator I ($18 to $20): Learn our systems, own the schedule, and keep files and credentials current Care Coordinator II ($21 to $22): Take on client home visits, onboarding, and process improvement with less oversight Operations Lead ($23 and up): Lead coordination across the agency as we grow Each step is tied to clear results you control, such as no uncovered authorized hours, same day punch corrections, and no lapsed certifications. You'll always know exactly what earns the next raise. What success looks like in your first year No client goes without a caregiver New caregivers start with complete files and meet their client before the first shift Missed punches are corrected the day they're found Caregiver certifications never lapse Caregivers stay because they feel supported, and you're a big reason why Why Rising Star A small team where your work is seen and your ideas get used Direct access to leadership with no three week waits for a decision Training from our nursing and operations leaders A real title and real responsibility that build your career How to apply Apply through Indeed with your resume. In your application message, tell us about a time you noticed something wasn't working in a client's care or a caregiver's schedule and you fixed it without being asked. What did you change, and what happened? We read every answer. That story tells us more than any resume. Rising Star Home Care Services is proud to be an equal opportunity employer.
