Job Description

at Assistance Plus in Waterville, Maine, United States Job Description Salary Range $27.00 - $29.50 Hourly Position Type Full Time Description The scope of practice for this position is to provide professional care coordination service to eligible clients. The Care Coordinator (CC) is an advocate for clients when securing services and is responsible for coordinating all services deemed necessary to meet the needs of clients served. The CC will be sensitive toward others and enjoy working with children and adolescents and work effectively within an interdisciplinary team. This position will be required to possess the ability and desire to work flexible hours to meet client demand. Because the CC will occasionally drive to a client's home in order to provide services, proof of vehicle insurance at $50K/$100K and a valid Maine driver's license is required.

Qualifications Position Requirements:

+ A minimum of three years relevant work experience working with children with a Serious Emotional Disturbance is required

+ Must be 18 years of age or older

+ Intermediate level computer skills

Education:

H as a minimum of a Bachelor's Degree from an accredited four (4) year institution of higher learning, with specialization in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing, or closely related field; OR who has a Bachelor's Degree from an accredited four (4) year educational institution in an unrelated field and at least one (1) year of full-time equivalent relevant human services experience; OR a who has Master's Degree in social work, education, psychology, counseling, nursing, or closely related field from an accredited graduate school
