This position provides administrative and member support to help ensure individuals receive timely, coordinated, and comprehensive behavioral health services. Key Responsibilities Assist Care Coordinators with daily care coordination and administrative tasks Schedule appointments, annual assessments, follow-ups, and appointment reminders Conduct member outreach, re-engagement, and follow-up calls Verify member demographics, insurance coverage, and authorizations Assist members with applications, documentation, and accessing community resources Coordinate referrals, transportation, and communication with healthcare providers and community agencies Prepare and organize documentation for annual reviews, treatment team meetings, referrals, and other services Monitor deadlines for assessments, authorizations, and required documentation Help maintain accurate and complete member charts Answer member calls and direct clinical concerns to appropriate staff Participate in treatment team meetings, departmental meetings, and required trainings Maintain member confidentiality and comply with HIPAA and agency policies Provide additional support to the Care Coordination team as needed Qualifications High school diploma or GED required Associate degree preferred Bachelor's degree in Social Work, Sociology, Psychology, Criminal Justice, or a related field strongly preferred Valid driver's license with an acceptable driving record Reliable transportation and ability to transport members as needed Ability to work effectively with individuals experiencing mental illness, substance use disorders, intellectual/developmental disabilities, and co-occurring conditions Strong organizational, communication, and time-management skills Ability to manage multiple priorities and work effectively within a multidisciplinary team

From $33,000.00 per year
