About the Role The Care Coordinator is the heart of our client care operations—ensuring each client receives consistent, high-quality care tailored to their needs and lifestyle. You'll work closely with families, caregivers, and community partners to create seamless, well-supported care plans. This role blends organization, heart, and leadership to keep our clients safe, supported, and comfortable in their homes. Key Responsibilities Serve as the primary point of contact for new client referrals and care transitions Coordinate personalized in-home care plans with compassion and attention to detail Ensure continuity and consistency of care by matching clients with the right caregivers Monitor client satisfaction and well-being through regular check-ins and in-home visits Drive throughout the service area to conduct client visits, caregiver check-ins, assessments, and community outreach with personal vehicle (mileage reimbursement). Maintain a valid driver's license, reliable transportation, and appropriate automobile insurance Support families with guidance, resources, and care-planning insight Supervise and support caregivers through regular communication, coaching, and performance evaluations Collaborate with healthcare professionals, insurers, referral partners, and internal teams to streamline services Participate in rotating on-call responsibilities as needed to maintain continuity of service Who You Are You are a naturally warm, highly organized professional with a heart for helping others. You're calm under pressure, skilled at juggling priorities, and comfortable working both independently and collaboratively.
Qualifications Education :
High school diploma or GED required; college degree or relevant certification preferred
Experience :
At least 2 years in a healthcare or home care setting Experience in care coordination or client services strongly preferred
Training & Skills Required :
