Attend weekly team meetings in assigned location to collaborate with providers.
Meet with Director of Access monthlyAssist support staff and clinicians in identifying appropriate referrals and resources for clients or potential clientsResponsible for care coordination, whether it involves coordination within CMMHC's programs or with another entity identified in the Certified Community Behavioral Health Clinic (CCBHC) statutory language related to care coordination.
Represent the agency in outreach activities by build strong ties with local groups, schools, and businesses.

The Care Coordinator is responsible for coordinating the care for clients and/or families receiving services, promoting quality care, and supporting collaboration for clients.
