We are a 132-bed non-profit skilled nursing facility governed by a volunteer Board of Directors. Our vision is to be a leader in the pursuit of healthcare excellence, the preferred choice for nursing & rehabilitative care, and a highly sought-out employer to healthcare professionals. We are NOT your typical nursing home! We put an emphasis on resident individuality, independence, and social well-being while providing our residents with care 24 hours per day, 7 days a week, 365 days per year. What are we looking for? We are seeking a dedicated care coordinator to join our team. The Care Coordinator is responsible for coordinating resident care by developing, implementing, monitoring and evaluating the interdisciplinary care of each resident to maintain each resident's highest level of physical, social and psychological well-being. Your role as a
Care Coordinator:
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Manage the care coordination of residents by developing, monitoring and evaluating interdisciplinary care. Conduct and guide residents and family members through admission process. Provide on-going advocacy and support for resident and resident families to ensure the preservation of rights and continuous flow of information. Participates in regular interdisciplinary meetings as a resource to enhance continuity and effectiveness of quality resident care. Assist and document resident and resident family quality assurance concerns. Coordinate training and education of all staff regarding departmental initiatives. Promote a positive image and relationships with vendors and the community internally/externally through special programs and community events to ensure exposure and visibility. Provide back up to team members; as needed. Required Qualifications Bachelor's degree in Social Work (BSW), or a related human services field. One (1) year of supervised experience in a health care setting. Valid driver's license with Class E endorsement; as required. Work Hours (expected): This position is a full-time position; hours of work are Monday through Friday 8:00 a.m. to 5:00 p.m. with some weekend, evening and night hours as needed.
