Job Description Do you love making a difference in the lives of our elderly population? Bishop Spencer Place is seeking a Social Services Coordinator to join our team! The Social Services Coordinator is responsible and accountable for assessing the psychological social emotional cultural or financial needs that impact a resident's health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community. Works with the multidisciplinary team to provide services that impact patient health and recovery. Provides assistance to Social Work team. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community. Full-time Monday through Friday 8 a.m. - 5 p.m. Assists the multidisciplinary team and patients with making discharge arrangements. Works with community agencies to ensure patients have the correct services in place prior to discharge. Assists patients in making transportation arrangements. Participates in treatment team meetings. Bachelor's required Previous experience in senior living preferred
