Job Description

We are seeking an experienced and compassionate LPN or RN to serve as Healthcare Coordinator for our Assisted Living community. This is a leadership position responsible specifically for the healthcare operations of our Assisted Living residents and healthcare team. The Healthcare Coordinator provides clinical oversight, coordinates resident care, supervises healthcare staff, and serves as a key point of communication between residents, families, physicians, pharmacies, and other healthcare providers. We are looking for a nurse who combines strong clinical judgment with excellent leadership, organization, and communication skills. The ideal candidate is comfortable managing a busy healthcare department, addressing resident needs as they arise, and holding staff accountable to consistent standards of care. Key Responsibilities Oversee the day-to-day healthcare operations for the Assisted Living community Assess new and existing residents and ensure healthcare needs are appropriately identified and addressed Develop, implement, review, and update individualized resident service plans and healthcare documentation using the community's electronic medical record system Monitor residents for changes in condition and coordinate appropriate interventions, physician notification, treatment, or higher levels of care when necessary Evaluate and follow up on falls, incidents, changes in condition, and other resident healthcare concerns Communicate effectively with residents, families, physicians, pharmacies, home health, hospice, and other outside healthcare providers Provide clinical guidance and support to healthcare staff while promoting sound judgment and consistent resident care Supervise, educate, coach, and hold healthcare team members accountable for performance and adherence to community policies and proceduresOversee healthcare department staffing and scheduling to maintain appropriate coverage Participate in hiring, orientation, training, and performance management of healthcare staff Ensure required healthcare documentation, assessments, service plans, and follow-up are completed accurately and timely Promote compliance with applicable assisted living regulations, community policies, and standards of care Work collaboratively with the Executive Director and other department leaders to address resident, family, staffing, and operational concerns Respond effectively to urgent resident-care situations and unexpected staffing challenges Participate in an evening and weekend on-call rotation as required Foster a professional, compassionate, and resident-centered culture throughout the Assisted Living healthcare department Qualifications Current LPN or RN license in good standingPrevious nursing leadership, management, or supervisory experience required Experience in assisted living, senior living, geriatrics, long-term care, or a related setting strongly preferred Strong resident assessment, clinical judgment, and triage skills Demonstrated ability to lead and manage healthcare staffExcellent communication and interpersonal skills, particularly when working with residents and families Strong organizational skills with the ability to prioritize multiple responsibilities and follow through on issues Dependable, professional, and able to remain calm and make sound decisions under pressure Comfortable working in a fast-paced environment where resident needs and priorities can change throughout the day Experience with PointClickCare or a similar electronic medical record system is a plus What We're Looking For This position is best suited for a nurse who enjoys leading people, solving problems, coordinating care, and building relationships with residents and families.
