We are seeking a compassionate, organized, and highly motivated Linkage to Care Specialist to join our Ryan White Part B team. Position Summary The Linkage to Care Specialist provides short-term, intensive support to individuals who are newly diagnosed with HIV or who have disengaged from HIV medical care. The primary goal of this position is to help individuals successfully enter or return to HIV medical care and connect them with appropriate supportive services. This is an active, client-centered position requiring consistent outreach, follow-up, problem-solving, care coordination, documentation, and collaboration with healthcare providers and community partners. The successful candidate will be comfortable working independently, managing multiple time-sensitive referrals, engaging individuals who may be difficult to reach, identifying barriers to care, and taking appropriate action to move each referral toward a successful outcome. Key Responsibilities The Linkage to Care Specialist will: Conduct outreach and engagement with individuals newly diagnosed with HIV and individuals who have fallen out of HIV medical care. Respond to linkage and re-engagement referrals within established program timeframes. Manage an active short-term caseload and maintain current knowledge of each client's status, barriers, appointments, outstanding needs, and next steps. Build rapport and establish trust with clients using person-centered, strengths-based, trauma-informed, and culturally responsive approaches. Identify barriers that may prevent clients from accessing medical care, including transportation, housing instability, financial hardship, insurance or benefits concerns, stigma, communication difficulties, and other challenges. Assist clients with scheduling initial or return HIV medical appointments and coordinate support necessary to help them access care. Provide education regarding HIV care, treatment, Ryan White services, and available community resources. Coordinate referrals to healthcare providers, behavioral health services, case management, and other supportive services. Follow up on medical appointments and referrals to determine whether services were successfully accessed. Promptly follow up with clients who miss appointments or remain at risk of not entering or returning to care. Use appropriate outreach strategies when clients are difficult to reach while maintaining confidentiality and professional boundaries. Identify cases requiring additional intervention and appropriately communicate significant barriers or concerns to the supervisor. Verify successful linkage or re-engagement in HIV medical care. Coordinate warm handoffs to Ryan White Case Managers or other providers when clients require ongoing services. Maintain accurate, complete, and timely documentation in CAREWare and other required systems. Track outreach attempts, client contacts, referrals, appointments, barriers, interventions, and linkage outcomes. Participate in team meetings, case conferences, chart reviews, quality improvement activities, audits, and program monitoring. Collaborate with ASC programs, healthcare providers, testing sites, community organizations, and other referral partners. Participate in community-based outreach and other program activities as needed. Minimum Qualifications Bachelor's degree in Social Work, Public Health, Human Services, or a related field required; a degree in Social Work is preferred. 1 - 2 years of experience in case management, outreach, public health, HIV services, care coordination, or a related setting preferred. Experience working with individuals living with HIV and/or populations experiencing barriers to healthcare strongly preferred. Experience with community outreach, client engagement, care coordination, or navigation of health and social services preferred. Valid driver's license and ability to travel as required. Technology Skills Candidates should possess basic technology skills and be comfortable independently using computers for daily work responsibilities, including: Microsoft Outlook, Teams, Word, Excel, and PowerPoint. Professional email communication, electronic calendars, and meeting scheduling. Electronic documents, shared files and folders, and online forms. Basic data entry, organization, sorting, and review in Excel. Electronic case management and data systems. Experience with CAREWare is preferred but not required. Candidates must be willing and able to learn CAREWare and other systems used by ASC and the Ryan White Part B program. Work Environment This position may require frequent local or regional travel for outreach, client engagement, appointments, and community activities. Occasional evening or weekend work may be required based on program and client needs. The Linkage to Care Specialist regularly works with individuals who may be experiencing a new HIV diagnosis, disengagement from healthcare, stigma, housing or financial instability, and other significant life challenges. Professionalism, compassion, confidentiality, sound judgment, and appropriate professional boundaries are essential. Performance Expectations Success in this position will be measured by timely and successful linkage and re-engagement outcomes, effective management and follow-up of assigned referrals, client engagement, documentation quality and timeliness, compliance with program requirements, communication, collaboration, and consistent follow-through. The Linkage to Care Specialist is expected to independently manage routine day-to-day responsibilities associated with assigned linkage and re-engagement cases while seeking supervisory guidance when situations require additional support or intervention.
