We are seeking an experienced and dependable Care Coordinator to join our home care team. This is primarily an administrative and coordination position . The Care Coordinator will manage caregiver schedules, conduct client intakes, review care plans, communicate with clients and families, interview caregivers, and help resolve day-to-day staffing and client needs. Responsibilities Schedule and coordinate caregivers and clients Conduct client intake and care plan reviews Communicate with clients, families, and caregivers Interview and help onboard caregivers Handle call-outs and arrange replacement coverage Maintain client and caregiver records Address client and caregiver concerns Occasionally provide direct care when a last-minute call-out requires coverage Qualifications Previous professional caregiving experience required CNA, PCA, HHA, or similar credential preferred Strong communication and organizational skills Reliable, compassionate, and able to problem-solve Home-care or care-coordination experience preferred Reliable transportation and ability to pass required background checks If you're an experienced caregiver looking to move into a coordination and leadership role, we'd love to hear from you.
