Job Description
CARE COORDINATOR - VETERAN DIRECTED CARE
Part-Time ~ 30 hrs/week ~ $21.08/hour Join our team and use your experience to coordinate care for qualified veterans who need supportive services to continue living independently in their own homes and community. The VDC Care Coordinator will be responsible for:- Assessing and orienting eligible veterans
- Preparing and monitoring service plans and budgets
- Assisting veterans as needed to hire care attendants and budget for other supportive goods/services
- Acting as liaison between the clients and the VA and the program's Financial Management Service agency The successful candidate will preferably have experience providing care coordination to veterans, older adults or people with disabilities, as well as strong computer and documentation skills.
