Home Care Coordinator Manager Come Join TREK Healthcare! We're growing and hiring! We are seeking an organized, dependable, and proactive Full-time Home Care Coordinator Manager to join our team. This position will be scheduled 40 hours per week, working Monday-Friday with a 30 minute lunch break. The Home Care Coordinator Manager is responsible for overseeing the day-to-day coordination and staffing of TREK Healthcare's home care services. This position ensures patients are appropriately and accurately staffed with Caregivers/CNAs while balancing patient needs, continuity of care, employee availability, and changing schedules. This role serves as the primary operational point of contact for our CNAs, providing support with scheduling needs, coverage, call-outs, vacations, and day-to-day operational concerns. The Home Care Coordinator Manager will work closely with the Home Health Director, RNs, other healthcare professionals, and the Executive Administrator to ensure patients receive consistent, reliable, and high-quality care. The ideal candidate is highly organized, communicates effectively, is comfortable making staffing decisions, and can manage multiple priorities in a fast-paced healthcare environment. Key Responsibilities Home Care Scheduling & Staffing Manage the daily and ongoing staffing schedule for home care services Ensure patients are accurately and appropriately staffed based on their authorized services, care needs, schedules, and preferences Coordinate caregiver/CNA assignments while prioritizing continuity of care whenever possible Balance patient staffing needs with employee availability, qualifications, requested schedules, and other scheduling considerations Monitor schedules to identify and address potential staffing gaps before they impact patient care Adjust schedules as patient needs, employee availability, or service requirements change Coordinate coverage for employee call-outs, vacations, planned absences, and other scheduling needs Respond to urgent staffing needs and make appropriate decisions to maintain patient coverage Maintain accurate scheduling records and communicate schedule changes promptly to patients, families, and staff as appropriate CNA Support & Oversight Serve as the primary operational point of contact for CNAs Assist CNAs with scheduling questions, coverage needs, and other day-to-day operational concerns Help resolve scheduling conflicts and employee concerns in a timely and professional manner Provide direction and support to CNAs regarding scheduling expectations and operational procedures Monitor staffing patterns and identify opportunities to improve efficiency, continuity, and employee scheduling Communicate staffing concerns or employee performance issues to the appropriate leadership team member Conduct and facilitate monthly CNA meetings Promote clear communication, accountability, teamwork, and professionalism among the CNA team Care Coordination & Communication Work closely with the Home Health Director, RNs, and other healthcare professionals to coordinate patient staffing needs Communicate changes in patient needs that may affect staffing or scheduling Collaborate with clinical leadership to ensure staffing assignments are appropriate for the patient's needs Serve as a communication link between Home Care staff and organizational leadership regarding operational and staffing matters Communicate professionally and compassionately with patients, families, caregivers, and members of the interdisciplinary team Maintain patient confidentiality and comply with organizational policies and applicable healthcare regulations Qualifications Required High school diploma or equivalent Previous experience in healthcare scheduling, home care, staffing coordination, or a related field Strong organizational and time-management skills Ability to manage multiple priorities and make sound decisions in a fast-paced environment Strong communication and interpersonal skills Ability to work independently while collaborating effectively with clinical and administrative leadership Proficiency with computers and electronic scheduling/documentation systems Ability to handle confidential patient and employee information appropriately Preferred Previous supervisory or management experience
