Charlotte, NC 28205Pay Rate:
$20-$22 per hour (Pay is dependent on experience)Hours:
Monday-Friday, 8:00 AM-5:00 PM About the Role We are seeking a compassionate, organized, and detail-oriented Care Coordinator to join our healthcare team in Charlotte, NC. This role is responsible for helping members navigate the healthcare system, coordinating referrals and services, monitoring care plans, and addressing barriers that may prevent members from receiving appropriate care. The ideal candidate will have a strong understanding of the healthcare ecosystem, excellent communication skills, and a genuine passion for helping senior members successfully achieve their care goals. Key Responsibilities Collaborate with an interdisciplinary care team to understand each member's individual needs, identify gaps in care, and address barriers to treatment. Coordinate referrals to external providers, vendors, specialists, and community services to ensure comprehensive care needs are met. Maintain accurate, timely, and compliant documentation related to member referrals and care coordination activities. Build and maintain strong, trusting relationships with members and their families to facilitate successful care coordination. Implement activities outlined in member care plans and provide ongoing monitoring to ensure appropriate follow-up and progress toward established goals. Maintain strict compliance with HIPAA standards and confidentiality requirements related to protected health information (PHI). Facilitate clear communication between internal care teams and external treating clinicians to support positive member outcomes. Identify and help resolve barriers to care, including healthcare system challenges, financial limitations, transportation needs, interpreter services, and prescription assistance programs. Serve as a patient navigator and primary point of contact for members and families, providing support, answering questions, and addressing concerns. Perform additional duties and responsibilities as assigned. Qualifications Strong understanding of the healthcare ecosystem, preferably with experience in healthcare benefits and insurance authorization . Genuine passion for helping senior members successfully follow and achieve their care plans. Ability to work effectively with diverse populations, including clinic staff, providers, members, family members, insurance carriers, vendors, and the general public. Proficiency in basic medical terminology, CPT, and ICD-10 coding . Solid understanding of Medicare criteria for medical equipment and supplies . Strong working knowledge of specialists, healthcare providers, and community resources. Exceptional communication, interpersonal, and rapport-building skills within an interdisciplinary environment. Proven ability to multitask, prioritize competing responsibilities, and maintain a high level of attention to detail. Proficient computer skills, including Microsoft Office Suite . Strong organizational and problem-solving skills. Ability to work independently while contributing effectively to a collaborative care team.
