Job Description

Liberty Cares With Compassion At Liberty Hospice we understand the unique needs of our patients and families facing terminal illness. That is why Liberty Hospice provides our hospice patients with state-of-the-art care and pain management services, delivered by our specially trained staff with emphasis on strength, dignity and compassion. We are currently seeking an experienced:
HOSPICE BEREAVEMENT COORDINATOR
Full Time Job Description:
Assesses the bereaved prior to and after the death of the patient. Manages the bereavement services program utilizing Hospice staff and volunteers. Plans, implements and facilitates bereavement support groups for hospice families and the community. Provides monthly reporting of bereavement services to Psychosocial Coordinator, Operations Manager and Patient Care Coordinator. Assists in providing community education on end of life issues as appropriate. Educates staff on ongoing grief resources, education and issues for increased knowledge.
Job Requirements:
Bachelor's degree in human services field such as social work, psychology, counseling or related field with experience and education in death and dying and the grief process; Masters degree preferred. Proficiency in writing and oral communication. Knowledge and skill with computer software programs to include Word, Excel. Good organizational skill and the ability to work efficiently with people. Experience in group facilitation and support intervention. Knowledge of the stages and reactions to grief and grief work. Visit www.libertyhomecare.com for more information.
