  • Saturday đź•’ Full-Time | Comprehensive Benefits Package 🏥 Substance Use Disorder Treatment | Aftercare & Discharge Planning Help Clients Build a Stronger Path Forward Silver Ridge with Pyramid Healthcare is seeking a compassionate and organized Aftercare Coordinator to join our team in North Carolina.
The Aftercare Coordinator plays a critical role in helping clients transition successfully from treatment into the next stage of their recovery. This position works directly with clients, families, counselors, referral sources, and community providers to develop comprehensive aftercare plans, coordinate referrals and placements, identify safe housing, and connect clients with ongoing treatment and recovery resources. If you are passionate about helping individuals with substance use disorders navigate the transition from treatment to continued care, this is an opportunity to make a meaningful impact every day. What You'll Do Provide quality direct and indirect client care to individuals with substance use or chemical dependency concerns Assess clients' continuing care, discharge, and referral needs Establish rapport with clients and encourage active participation in aftercare planning Obtain necessary information and releases from clients, families, referral sources, and other appropriate resources Make referrals to mental health and drug and alcohol treatment facilities for continuing care Coordinate Halfway House interviews and placements as appropriate Work closely with counselors and members of the multidisciplinary treatment team to coordinate client aftercare needs Assist clients in identifying safe housing and appropriate treatment and recovery resources following discharge Develop and coordinate comprehensive aftercare plans based on identified client needs Communicate with referral sources, treatment providers, and other appropriate resources to facilitate continuity of care Attend and participate in program, clinical, and administrative meetings Actively support the facility's ongoing referral program Support organizational efforts related to client transitions, continuity of care, census, and program goals Maintain timely and accurate documentation related to aftercare planning, referrals, placements, and client coordination Maintain confidentiality and exercise discretion regarding all company and client information Complete all required training as designated by Pyramid Healthcare and applicable accreditation/licensing entities Perform other duties as assigned Qualifications Required High School Diploma required Bachelor's Degree in a human service-related field preferred Knowledge, Skills & Abilities The ideal candidate will have: Excellent oral, written, and interpersonal communication skills Knowledge of clinical levels of care, primary funding streams, and referral sources Strong problem-solving skills with the ability to gather and analyze information Ability to respond appropriately and effectively in crisis situations Working knowledge of federal and state standards, as well as applicable regulatory and accreditation standards Strong client engagement and rapport-building skills Knowledge of aftercare planning, continuing care services, and community-based treatment resources Ability to identify appropriate mental health, substance use disorder, housing, and other continuing-care resources based on client needs Ability to coordinate referrals, interviews, placements, and transitions between treatment settings Ability to communicate and collaborate effectively with clients, families, counselors, multidisciplinary treatment teams, referral sources, and outside providers Strong organizational and time-management skills with the ability to coordinate multiple client needs and referrals Ability to maintain accurate and timely documentation Ability to exercise discretion, maintain confidentiality, and demonstrate appropriate professional boundaries Ability to work effectively both independently and as part of a multidisciplinary team Work With Purpose At Silver Ridge with Pyramid Healthcare , you'll have the opportunity to make a direct impact on individuals working toward recovery.
