Job Description

Stedman-Wade Health Services, Inc. (SWHS) is a Federally Qualified Health Center (FQHC) committed to providing compassionate, quality healthcare to patients in Cumberland County and surrounding areas. The Licensed Clinical Social Worker-Associate (LCSW-A) provides integrated behavioral health services as part of a multidisciplinary care team. The LCSW-A offers assessments, brief interventions, counseling, and referrals to help patients manage emotional, behavioral, and psychosocial challenges that impact their overall health. The LCSW-A will also provide assistance to the care management team by working collaboratively to support patient care plans and needs. Key Responsibilities Provide evidence-based short-term counseling and crisis intervention to patients across the lifespan. Conduct psychosocial assessments and develop individualized care plans. Collaborate with primary care providers and other clinical team members to support patient treatment goals. Engages with patients to identify and address barriers that impede health outcomes Facilitate referrals to specialty mental health and community resources as needed. Maintain accurate and timely clinical documentation in the electronic health record (EHR) and the care management software. Participate in care team meetings, case reviews, and quality improvement initiatives. Support patient engagement in preventive care, chronic disease management, and overall wellness.
