Job Description

Health Connect America, Inc. seeks an Integrated Care Extender to support delivery of community based behavioral health services. In this role, you'll assist clinicians with care coordination, client engagement, and follow up for children, adolescents, adults, and families. Responsibilities include scheduling and outreach, supporting assessments and treatment plan implementation, documenting services, linking clients to community resources, and collaborating with a multidisciplinary team. You'll help ensure client centered, trauma informed care that promotes safety, recovery, and improved outcomes in homes, offices, and community settings. Responsibilities Support clinicians with care coordination and follow-up for children, adolescents, adults, and families. Assist with client intake, appointment scheduling, reminders, and outreach. Document services, contacts, and client progress accurately in EHR systems. Help implement treatment and service plans under licensed or qualified staff direction. Link clients and families to community resources and support services. Participate in multidisciplinary team meetings and case reviews. Monitor client engagement and report safety or risk concerns promptly. Maintain client confidentiality and adhere to HIPAA and organizational policies. Provide logistical support for in-home, in-office, and community-based visits. Contribute to a compassionate, client-centered, and culturally responsive care environment.
