We are seeking a compassionate and skilled Memory Care Coordinator to join our team at Grand Villa Assisted Living in Winston-Salem, North Carolina. As a leading provider of assisted living services, we are dedicated to delivering exceptional care and support to our residents. Our Memory Care Coordinator will play a critical role in ensuring the well-being and quality of life of our residents living with memory impairments.
MUST HAVE A NORTH CAROLINA MEDICATION TECHNICIAN CERTIFICATION.
About theRole:
As a Memory Care Coordinator , you will be responsible for coordinating and delivering personalized memory care services to our residents.This includes:
Developing and implementing individualized care plans in collaboration with the care team Providing emotional support and guidance to residents and their families Facilitating resident activities and socialization programs Collaborating with caregivers and other healthcare professionals to ensure seamless care transitions Maintaining accurate and up-to-date records of resident care and servicesResponsibilities:
Coordinate and deliver memory care services, including medication management, behavioral interventions, and daily living assistance Develop and maintain relationships with residents, families, and care staff to ensure effective communication and collaboration Participate in interdisciplinary team meetings to discuss resident care and progress Identify and report any changes in resident condition or concerns to the care team Maintain a clean, organized, and safe environment for residentsRequirements:
High school diploma or equivalent required; degree in a related field (e.g., gerontology, healthcare administration) preferred Minimum 0 years of experience in a long-term care or memory care setting Excellent communication and interpersonal skills Ability to work effectively in a fast-paced environmentWhat We Offer:
