About the

Role:

As a Memory Care Coordinator , you will be responsible for coordinating and delivering personalized memory care services to our residents.

This includes:

Developing and implementing individualized care plans in collaboration with the care team Providing emotional support and guidance to residents and their families Facilitating resident activities and socialization programs Collaborating with caregivers and other healthcare professionals to ensure seamless care transitions Maintaining accurate and up-to-date records of resident care and services

Responsibilities:

Coordinate and deliver memory care services, including medication management, behavioral interventions, and daily living assistance Develop and maintain relationships with residents, families, and care staff to ensure effective communication and collaboration Participate in interdisciplinary team meetings to discuss resident care and progress Identify and report any changes in resident condition or concerns to the care team Maintain a clean, organized, and safe environment for residents

Requirements:

High school diploma or equivalent required; degree in a related field (e.g., gerontology, healthcare administration) preferred Minimum 0 years of experience in a long-term care or memory care setting Excellent communication and interpersonal skills Ability to work effectively in a fast-paced environment
