Care Coordinator We are seeking a reliable, detail-oriented, and compassionate Care Coordinator to support clients, families, and caregivers while ensuring consistent, high-quality home care. This role combines scheduling, care coordination, client support, and hands-on caregiving when needed . Key Responsibilities Build and maintain strong relationships with clients, families, physicians, and caregivers. Develop and manage individualized care plans, client goals, and caregiver training. Monitor caregiver notes, track client progress, and communicate changes or concerns. Manage and fill open shifts using AxisCare , matching caregivers to client needs. Provide on-call support during business hours, evenings, weekends, and emergencies. Provide transportation in a company vehicle and accompany clients to medical appointments when needed. Train and support caregivers to meet agency standards. Step in to provide direct care when coverage is needed, including personal care, housekeeping, meal preparation, shopping, companionship, and transportation/escorts. Qualifications Prior experience in home care, healthcare, care coordination, or a fast-paced office environment preferred. Friendly, outgoing, compassionate, and professional personality. Bilingual English/Spanish preferred. Active CNA license preferred. Strong communication, organization, problem-solving, and multitasking skills. Ability to remain calm and resourceful in time-sensitive situations. Comfortable using AxisCare, Microsoft Office, Windows computers, and standard office equipment . Ability to lift up to 30 lbs. and drive a company vehicle as needed.
