Job Description
Brief Enhanced Care Coordinator About the Role The BrECC Coordinator provides intensive, short-term care coordination for children and youth (ages 0-21) and their families who are experiencing emerging or escalating behavioral health concerns. Using a strengths-based, family-driven, and trauma-informed approach, you'll partner with families to assess their immediate needs, develop focused care plans, coordinate services across multiple systems, strengthen natural supports, and help ensure successful transitions to ongoing community resources. This is meaningful, hands-on work that makes a real difference for families navigating complex systems during a critical time. What You'll Do Provide short-term care coordination in the family's home, community, school, healthcare setting, or virtually Build trusting relationships with children, youth, and families using strengths-based, family-driven, and trauma-informed practices Complete screenings and assessments to identify family strengths, behavioral health needs, and service priorities Help families set goals and develop individualized care coordination plans Coordinate services with behavioral health providers, medical providers, schools, child welfare, and other community partners Connect families to community resources and help them navigate complex service systems Facilitate communication among providers to improve coordination and reduce duplication of services Complete all required documentation, assessments, and progress notes within established timelines Maintain client confidentiality Maintain an up-to-date calendar of appointments Participate in ongoing professional development and required training Participate in cross-training as assigned to help meet staffing needs across programs Attend staff meetings and weekly reflective supervision Adhere to Family Resource Center policies and procedures What We're Looking For Experience working with children, youth, and families using strengths-based, family-centered approaches Knowledge of behavioral health systems and community resources preferred Ability to build collaborative relationships with families and community partners Strong communication, organization, and documentation skills Experience working with culturally diverse populations Working knowledge of computers and electronic documentation systems Ability to communicate effectively with families, providers, schools, funders, agency staff, and community partners Education and Experience High school diploma required Minimum of two years' experience working with families and children in a social service, home health, or other early childhood program setting Physical Requirements Ability to lift up to 10 pounds Sitting most of the time, with some bending and reaching Periodic standing, walking, and bending Repetitive movements of wrists, hands, and fingers for typing and writing Close and distance vision required Benefits Paid Time Off Paid Holidays Paid FMLA Flexible Schedule Health and Dental Insurance
