Join our talented multi-disciplinary team in our Child and Family Programs working with children and families experiencing symptoms of mental illness that impact functioning across settings. As a Care Coordinator, you'll work closely with our Clinical Team to provide care coordination and case management services for a large caseload of clients. The primary role of the Care Coordinator will be to assess needs, coordinate care and make referrals to community resources for assigned clients and families.
Essential Responsibilities:
  • Help clients and families understand treatment options and make informed decisions
  • Work with treatment team to facilitate case coordination and information sharing
  • Participate in the development and implementation of treatment plans and other clinical documentation
  • Promote effective utilization of clinical resources, community resources and other social supports
  • Care coordination/communication both internally and externally Additional Information Help for Additional Information.
Education:
  • Associate's degree in Social Work, Rehabilitation, Psychology, Education or related human services field with 2 years working with person(s) who have severe mental disability or understanding of mental illness acquired through provision of support to family members with mental illness.
  • Bachelor's degree in a relevant field
Experience:
  • 2 years' case management experience preferred.
Required Skills:
  • Excellent organizational and communications skills.
  • Must be flexible and demonstrate the ability to work with minimal supervision.
  • Excellent customer service skills.
  • Excellent computer skills and knowledge of Microsoft Office.
  • Ability to learn necessary computer and technical skills essential to the position and Agency needs.
  • Ability to relate to staff, clients and community around a mental health system.
  • Maintain skill competencies in customer service, quality improvement, compliance and safety.
Preferred Skills:
  • Knowledge of and experience with an electronic medical record system
  • Prior experience in a community mental health environment
Physical Requirements:
  • Ability to access all Agency sites
Other:
  • Ability to obtain and maintain Agency and/or State of New Hampshire required background check(s).
  • Understanding of the importance of confidentiality and the ability to maintain that confidence.
  • Ability to travel in connection with the job duties assigned.
  • Have and maintain a valid state driver's license.
  • Have reliable transportation.
  • Have and maintain personal automobile insurance.
