Job Description

About Us The South Jersey AIDS Alliance is a caring, compassionate organization dedicated to the fight against HIV/AIDS, aiming to improve health outcomes and reduce barriers to care for individuals and families in our community. We provide client-centered services that promote dignity, equity, and well-being. About the Role As a Community Care Coordinator, you will play a key role in connecting clients to the medical, dental, and social services they need to thrive. You'll provide individualized case management, advocate for client needs, and work closely with healthcare providers and community partners to ensure clients receive comprehensive, coordinated care.
  • Role may include occasional evening/weekend shifts What You'll Do
  • Conduct intake assessments and develop individualized service care plans.
  • Coordinate and track medical, dental, and social service appointments.
  • Facilitate referrals and follow up to ensure access to services.
  • Assist clients with applications for benefits and supportive services.
  • Document client interactions and maintain accurate records in accordance with agency and funding requirements.
  • Respond to client emergencies and provide crisis intervention when needed.
  • Collaborate with providers, case managers, and community agencies to support client success.
  • Provide culturally competent services to a diverse client population.
  • Maintain compliance with HIPAA and agency policies regarding confidentiality. What We're Looking For
  • Associate's degree in human services or a related field required; bachelor's degree preferred.
  • 1-3 years of experience in human services or a related field strongly preferred.
  • Spanish speaking a plus!
  • Strong organizational, problem-solving, and decision-making skills, with the ability to prioritize competing demands and meet deadlines.
  • Excellent attention to detail and accuracy in documentation and client records.
  • Knowledge of cultural and lifestyle diversity issues; familiarity with HIV/AIDS and related health and social issues a plus.
  • Working knowledge of local healthcare providers, social service agencies, and public entitlement programs preferred.
  • Strong written and verbal communication skills with the ability to interact effectively with clients, colleagues, and community partners.
  • Demonstrated ability to build effective, respectful, and supportive relationships.
  • Ability to exercise sound judgment and maintain professionalism, integrity, and confidentiality.
  • Proficiency in Microsoft Office (Word, Excel, Outlook) strongly preferred.
  • Valid driver's license and ability to drive company vehicle required. Why You'll Love Working Here
