In this pivotal role, you will serve as a key liaison among hospital departments, patients, families, and community-based service providers to support coordinated, patient-centered care. The Hospital Liaison will facilitate communication, coordinate care plans, support effective discharge planning, and connect individuals with appropriate clinical and community resources. The position requires a proactive and professional approach to working with individuals with complex mental health, substance use, neurodevelopmental, and disability-related needs. Key Responsibilities Serve as a primary point of contact among hospital staff, patients, families, and external service providers to facilitate coordinated, comprehensive care. Coordinate and communicate information regarding psychiatric care, treatment progress, discharge planning, and follow-up services to promote continuity of care. Maintain accurate and timely documentation within electronic health record (EHR) systems, including patient interactions, treatment updates, referrals, and care coordination activities. Support individuals with co-occurring mental health and substance use disorders by helping integrate behavioral health services with appropriate addiction counseling and recovery resources. Advocate for individuals with neurodevelopmental disorders and disabilities and facilitate access to appropriate clinical, social, and community-based resources. Provide supportive counseling related to gender identity, gender expression, and other gender-related concerns in a respectful, inclusive, and patient-centered manner. Collaborate with multidisciplinary treatment teams to support the implementation of art therapy and other therapeutic interventions designed to promote patient engagement, emotional well-being, and recovery. Establish and maintain effective working relationships with internal departments and external agencies to ensure timely coordination of services. Promote patient-centered practices while maintaining confidentiality, professional standards, and applicable healthcare regulations. Required Skills & Qualifications Demonstrated experience in psychiatric or behavioral health care, with a strong understanding of mental health treatment approaches and care coordination. Knowledge of addiction counseling principles and experience supporting individuals with co-occurring mental health and substance use disorders. Proficiency with electronic health record (EHR) systems and a demonstrated ability to maintain accurate, comprehensive, and confidential documentation. Ability to provide sensitive, professional, and culturally responsive support related to gender identity, gender expression, neurodevelopmental conditions, and disabilities. Working knowledge of psychopharmacology principles and their relevance to psychiatric treatment and care planning. Excellent interpersonal, communication, and organizational skills, with the ability to effectively communicate complex information to patients, families, healthcare professionals, and community partners. Strong advocacy and problem-solving skills, particularly when working with individuals who have complex or specialized needs. Experience incorporating art therapy, creative therapeutic approaches, or other engagement strategies into behavioral health services is preferred.
